Abstract: This report is based on interviews with seven Thought Leaders who have a stake in anticoagulation therapy. The discussions took place in January 2010. This Panel continues where our last report left off, exploring the revolutionary use of antithrombotics and antiplatelets for use in the prevention of ischemic events during/after surgery, acute coronary syndrome, secondary prevention venous thromboembolism and secondary prevention for atrial fibrillation. Abstract: This report is based on interviews with eight Thought Leaders who have a stake in anticoagulation therapy, conducted immediately after the European Society of Cardiology 2009 meeting. On the antithrombotics front, the news at ESC was dominated by Pradaxa (dabigatran). Our Panel was very favorably impressed by the RE-LY data, but also points out meaningful ways for follow-on antithrombotics to compete in the SPAF (secondary prevention of stroke in patients with atrial fibrillation) market. According to our Panel, it is important to realize that SPAF is undertreated today and there will be plenty of room for follow-up drugs, either direct thrombin inhibitors or anti-FXa. The Panel next turns to platelet inhibition strategies for the treatment of ACS (acute coronary syndrome). In order to clarify their position on the roles of clopidogrel, prasugrel and ticagrelor, they dissect the CURRENT-OASIS-7 and PLATO trials, and make relevant comparisons and contrasts to TRITON.

Abstract: This report is based on a series of six new interviews with US Thought Leaders (emergency medicine, cardiothoracic surgery, cardiology), and supporting comments from our last panel. It focuses on unmet needs related to antithrombotic and antiplatelet therapy, and a rather large group of developmental therapies targeting acute, sub-acute and long-term indications for these two classes of drugs. Thromboembolic disease kills more people every year than lung cancer. As an oral antithrombotic, warfarin is the much despised standard of care. Sanofi-Aventis has established enoxaparin as the leading hospital based low molecular weight heparin. In the next two years each will face significant short-term competitive challenges from new orally available direct thrombin inhibitors (dabigatran) and factor Xa inhibitors (rivaroxaban). Clopidogrel has dominated the antiplatelet category for a decade. It too has notable limitations that are about to be exploited. See brochure for more.
